Cut and Paste the following to your insurance agent. For vendors with food sales, we are only able to accept yearly certificates (not one day event).

For our insurance, we need the one page certificate that additionally names entities below and follows these requirements:

□       Name Hacienda CDC; Portland Mercado LLC; Hacienda QALICB LLC as Additional Insured

□       $1 million in liability

□       At least $1,000,000 per occurrence (covering damage to premises and bodily injury)

□       Mailing address is 7238 SE Foster Road, Portland, OR 97206

□       Description of operations is located at 7238 SE Foster Road, Portland, OR 97206

Please email a pdf of your certificate to portlandmercado@haciendacdc.org so we can have our insurance agent approve the certificate.
